NDIS Eligibility Reassessments: 2025 Key Changes

Summary

An NDIS reassessment (previously known as a "plan review") is a scheduled check-in with the NDIA to determine if a participant's current supports are still appropriate, or if they need to be adjusted.

Introduction

An NDIS reassessment (previously known as a "plan review") is a scheduled check-in with the NDIA to determine if a participant's current supports are still appropriate, or if they need to be adjusted.

Reassessments can result in:

  • Continuation of the same funding and supports
  • Changes to the plan based on current needs
  • Transitions to longer-term or more tailored funding

It's a vital part of ensuring that participants receive the right support as their circumstances change.

What's New in 2025?

As of 2025, several key updates have been made to the reassessment process:

Simplified evidence requirements – Less repetitive paperwork is now needed, especially for ongoing and stable conditions.

Longer plan durations – Some participants may receive 3–5 year plans if their support needs are stable, reducing the burden of frequent reviews.

Functional capacity focus – The NDIA is placing greater emphasis on a participant's functional ability and day-to-day impact, not just clinical diagnosis.

Digital support tools – Participants can now use my NDIS Portal or mobile tools to track reassessment timelines, upload evidence, and book appointments more easily.

PACE alignment – Reassessments are now aligned with the PACE system (Provider and Connection Engagement), which streamlines coordination between participants, providers, and NDIA planners.

Why the Changes Matter

These updates aim to:

  • Reduce stress and administrative burden for participants
  • Make reassessments more person-centred and outcome-focused
  • Ensure that funding reflects real needs, not just diagnostic labels
  • Promote smoother planning and better use of NDIS resources

This shift is especially important for individuals with psychosocial or fluctuating disabilities, where the focus on day-to-day functioning gives a clearer picture of lived experience.

How to Prepare for Your Reassessment

To ensure the process goes smoothly, participants should:

    • Track progress – Keep a record of support outcomes, changes, or concerns.
    • Collect relevant evidence – This includes recent therapy reports, support worker notes (like progress notes), or allied health assessments.
    • Talk to your support coordinator or provider – They can help identify what supports are working and what may need to change.
    • Engage early – Start preparation 6–8 weeks before the reassessment is due.

Key Tips for a Smooth Reassessment

          • Stay organised – Don't leave evidence collection to the last minute.
          • Keep support notes – Daily progress notes help track needs and progress.
          • Be honest – Clearly communicate challenges, not just improvements.
          • Use your voice – It's your plan; your input matters.
          • Ask for help – Your provider or support coordinator can guide you through the whole process.
